Home & Facility Visitation Program

Loneliness and fatigue are two problems that affect quality of life for frail elders. Both can result in withdrawal, failure to comply with physician orders, and diminished quality of life. Our home & facility visitation program is designed to provide socialization and care oversight for home & facility bound elders.

Truth is, you did so much. From our initial meeting when you brought my mother and sister firmly but compassionately back to Earth by convincing them that staying at home was no longer an option for my parents, to your attending his memorial service, you were there for us. In between, there were the calls, the recon work, the hospital visits, the flowers, the listening to me – all of it so helpful. Thank you.

When we arrive on home or facility visits, we focus on their physical, social, emotional, and spiritual needs. We work hard to develop a trusting relationship. We invite conversation. When possible, we encourage clients to be active participants in their own care. During a facility visit, we ensure that promised services are delivered. Regardless of location, at a minimum, we monitor safety, perform a fall risk assessment, evaluate nutrition & oversee medication use.
