Senior Emergency On-Call Service Includes:
- Accompany the elder, from their home or facility, to the Emergency Department
- Provide ED medical personnel with a comprehensive medical history and a list of current medications
- Remain with the elder until treatment has concluded
- Communicate with designated family members throughout the experience
- Coordinate discharge activities, including transportation home
- Arrange for post-discharge care, as needed, and with family permission
