As an elder’s heath grows more fragile, the need for senior medical services intensifies. A frail elder is likely to receive care not only from the family physician, but also from several specialists. Multiple medical appointments in a single week are not unusual. Ensuring that all physicians are fully informed of symptoms, medications, and current problems can be difficult. Family members often find that unless their loved one is accompanied to an appointment, important information such as prescription changes may forgotten or ignored.

Our geriatric care managers will schedule and provide transportation to medical appointments. They will meet with doctors/clinicians, provide a list of current medications and describe symptoms and/or response to treatment. Care managers will drop off and pick up prescriptions and provide family members with a report following a physician visit. Senior medical care coordination can be overwhelming, but it doesn’t have to be.
