I think this a very good assessment of Dad’s current state of health and I find both the short-term and long-term recommendations right-on and practical.

We interview the client and their primary caregivers. If necessary, we talk with health professionals involved in care. Based on our findings, we develop a prioritized plan of action, and provide resources to address the concerns we identify.
Family members often use this comprehensive assessment as a starting point to reach a common understanding of their loved one’s plight and to build a consensus on how to best move forward.
