Getting Started with Home Care Services
All home care assignments start with a home visit by an RN care manager. She will:
- Conduct an interview with family and/or other caregivers to better understand the current situation and the hours of care desired
- Conduct a physical assessment and health status review
- Determine the prospective client’s current functional abilities (how much help they need with toileting, bathing, dressing, grooming, ambulating and eating)
- Describe the level of caregiver that will need to be assigned, based on assessment findings (read more about caregiver roles)
- Secure a contract for services
- Develop a detailed plan of care with the primary caregiver
Once the intake assessment is complete and a contract has been signed, the care manager will identify a caregiver(s) with appropriate skills, introduce them to you and your loved one, and provide a comprehensive orientation to ensure that care needs are understood. From that point forward, the care manager will work with you to ensure that the services you request and the services you receive are one and the same.
